Physical Therapist
Position Summary
Sea Coast Home Health is seeking a motivated and compassionate Physical Therapist (PT) to join our growing team in a Full-Time or Per Diem capacity. The Physical Therapist will provide skilled therapy services to patients in their homes, develop individualized treatment plans, and collaborate with an interdisciplinary team to optimize patient outcomes.
Responsibilities
- Evaluate patients and develop individualized treatment plans.
- Provide skilled physical therapy interventions in the home setting.
- Educate patients and caregivers on exercises, safety, and disease management.
- Complete timely and accurate documentation in accordance with agency, state, and federal requirements.
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate care.
- Participate in case conferences and quality improvement activities.
Qualifications
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist license in California.
- Current CPR certification.
- Home health experience preferred but not required.
- Excellent communication, organizational, and clinical skills.
- Reliable transportation and valid driver's license.
